Check the permissions and output button settings.
- Grant permissions to users for OFC_Template object and OFC_Child Object Option object.
- Check that the template API name exists in the OFC_Template record. The template API name is the value of the field "Template API Name" in the OFC_Template record, not the OFC_Template record Id.
- For the Lightning Component button, check that the template API name is set correctly in the "(Button XX) Template" of the ofcForRecordPage component on the object's Lightning record page.
Create buttons on the Lightning record page - For the Flow, check that the template API name is set correctly in the "template" parameter of the ofcForFlowScreen component.
